A code embedded in an email you are reading on the same phone cannot be scanned by that phone's camera, and screen glare defeats the attempt when you try from a second device. Codes inside PDFs behave the same way, especially at the small sizes used on boarding passes and event tickets.

A QR code cropped out of a low-resolution screenshot may simply not have enough pixels per module left for any decoder to resolve; re-exporting the source at a larger size usually fixes it instantly. Photographs taken at an angle can also fail because the square modules become trapezoids.
Heavy JPEG compression is another common culprit — the artefacts around high-contrast edges blur the module boundaries. Where you have the choice, save as PNG. Cropping tightly to the code, straightening a skewed photo, and increasing contrast in any image editor all help before you try again.
Reading before opening is a security habit
A QR code is opaque by design: nothing about the pattern tells a human where it leads, which is precisely what makes malicious codes effective. Stickers placed over legitimate codes on parking meters and restaurant tables are a well-documented tactic, and the victim never sees the destination until it is already loading.
Decoding an image first breaks that chain. You see the full URL as text, including the real domain and any redirect service in front of it, and you decide whether to visit it. For any code that arrived unsolicited — in an email, a letter, or a printed leaflet — that inspection step is worth the five seconds it costs.

Why does my photo of a poster fail to decode?
Usually angle or focus. Retake it square-on with the code filling the frame, or crop and straighten the existing photo. Extreme perspective distortion prevents the decoder from squaring up the module grid.
Does the decoder open the link automatically?
No. It shows the decoded content as text so you can inspect the destination first, which is the whole point of reading a code you did not create yourself.
Is my image uploaded to a server?
